I gave up sugar years ago, and went on a low carb diet after. I doubt that I "need" insulin much for the last few years, since I consume very little sugar and starch. Add to this story that insulin actually causes problems in the body.
So why would someone choose to die for lack of insulin when a better diet can avoid that? For example, start the morning with bacon and eggs, have a nice lunch of tuna fish salad, and have a modest meat dish for dinner.
Or one can insist on consuming carbs and die of "complications". It is a choice to die early.
